Incident Summary

On Thursday, June 18, 2026, we identified an issue where some customers were unable to sign in using SSO. After clicking “Sign in”, users were immediately redirected back to the login page.

Impact

Affected: eQMS users across APAC, EU, and US regions who use SSO (notably Azure Active Directory).

Symptoms: SSO sign-in loop/redirect back to the login page, preventing successful authentication.

Root Cause

A recent authentication service deployment introduced stricter cookie validation. The service could not process identity provider cookie values containing spaces, which impacted customers using Azure Active Directory for SSO.

Resolution

We rolled back the authentication service to the previous stable build at approximately 2:51pm EDT (America/New_York -04:00), restoring SSO login. We then re-tested SSO in production and pre-release and monitored to confirm stability.

Next Steps / Prevention
  • Restore an Azure AD test environment to validate SSO changes before release.

  • Track and remediate the cookie compatibility issue to prevent similar regressions.
