On Tuesday, June 30, 2026, we identified an issue preventing some customers from completing trainings in our eQMS product. A small number of EU-based organizations encountered errors when submitting training assignments.

Affected: A subset of EU-based organizations using eQMS.

Symptoms: Errors when submitting training assignments, preventing users from completing trainings.

A recent deployment to our training service introduced changes to file URL handling, which caused errors during training completion.

We rolled back the training service to the previous stable version, which restored training completion.

We completed follow-up work to improve file name encoding and URL handling so files with spaces or special characters are handled consistently.
